ADMISSION

    Qualification for Students:

• Age 1 -15 (during enrollment).
• Bring along copy of medical report from pediatrician and / or psychological report
  which should indicate diagnosis of the child.
• Fill in the registration form and attached the following :
        a. 1 Copy of Family Card
        b. 1 Copy of Child Birth Certificate
        c. 2 pieces of Students Photo (3 x 4 cm)
